Pain & Spine Institute is the local resource you can count on for your care. We offer state of the art facilities and minimally invasive procedures to help your get back to work and home to your families with less pain.

There are many patients who may benefit from a consultation with a pain management physician, specifically, patients who are experiencing acute or chronic pain or those patients who have had recent changes in physiological, functional, psychological or neuromusculoskeletal changes as a result of a painful condition, unusual reactions to treatment, or medication management.

New Patients

1. If you want us to call you to schedule a consultation, please email your contact information to referrals@pain-spine.com or use the “Book an Appointment” button located throughout our webpage. We will make every effort to return your call that day and, if we are unable to, we will call within 24 hours.

2. If you would like to speed up the registration process and streamline your first visit to our pain clinic, please download and complete our new patient form prior to your visit. You can find this form at the top of the webpage located under the “patients tab” or click below.

3. Bring any records and imaging that you may have to your every visit. Examples include: Imaging discs (X-ray, MRI, CT…), referral letters, lab work, or any previous care you have received.
