Targeting Vertebrogenic Low-Back Pain
Basivertebral nerve ablation, sometimes called BVN RFA, is a minimally invasive procedure for selected chronic low-back pain associated with vertebral endplate changes. The basivertebral nerve carries pain signals from within the vertebral body and can be targeted when the clinical and imaging findings support vertebrogenic pain.
Using image guidance, the physician advances a specialized probe into the targeted vertebral body. Controlled radiofrequency energy is then applied to interrupt pain signaling from the basivertebral nerve.
Treatment planning also considers the duration of symptoms, prior nonsurgical care, bone health, infection risk, other medical conditions, and the patient’s functional goals.
Match the MRI Findings to the Pain Pattern
MRI findings and the pain pattern must support vertebral endplate pain before treatment is considered.
Why It May Be Considered
Chronic pain centered in the lower back
A symptom pattern consistent with vertebrogenic pain
MRI findings that support vertebral endplate involvement
Insufficient improvement after appropriate nonsurgical care
